What is a medication review and how does it work?

A medication review is a second-opinion clinical service offered in the Hello Heart app. If your organization is participating in this feature, you will have the option to give your consent to opt-in. Then, based on your clinical qualification criteria, along with blood pressure readings and cardiovascular health history, a licensed pharmacist can evaluate your health data to provide personalized medication recommendations to help improve your cardiovascular health. 

The Care For Hearts pharmacist checks whether your current treatment follows evidence-based medical guidelines. After reviewing your information, the pharmacist may recommend a medication change, confirm that your current treatment appears appropriate, or determine that more information is needed.


Who is Care for Hearts?

Care for Hearts is an independent telehealth practice focused on preventive cardiovascular care. It pairs you with a team of preventive cardiologists, pharmacists and support staff to help you take control of your heart health. For more information, you can visit the Care for Hearts website.

What information does the pharmacist review?

During the review, a licensed pharmacist from Care for Hearts may take a look at:

  • Your blood pressure readings in the Hello Heart app.
  • Your heart rate readings.
  • The medications you entered in the app.
  • Basic information such as your age.
  • Available medical records from your healthcare providers about your cardiovascular-related health.

Looking at this information together helps the pharmacist better understand your current medications and heart health history. 

Please note: You do not need to schedule an appointment or start the review yourself.
 

When you qualify:

  1. A licensed Care for Hearts pharmacist will review your available health information.
  2. The pharmacist will check for possible gaps in your current medication treatment.
  3. The pharmacist then records the result of the review.
  4. When it’s been completed and a message is available, it will be sent to your Hello Heart app inbox.

 

What happens after the review?

Your medication review may have one of the following results:

  1. A recommendation is made: The pharmacist identifies a possible gap and recommends adding or changing a medication.
  2. No change is recommended: The pharmacist confirms that your current medication treatment appears appropriate.
  3. The review cannot be completed: The pharmacist does not have enough recent health information or medical records to complete the review.

A recommendation does not mean that your medication has already been changed. Continue taking your medications as directed unless a healthcare provider advises otherwise.

 

How will I know when my results are ready?

When the pharmacist sends you a message, a red indicator will appear on the Profile icon in the Hello Heart app.

To read the message:

  1. Tap on the Profile icon in the top right corner.
  2. Select the message notice that appears, or open your inbox.
    1. Your inbox will be an envelope icon at the top of the screen.
  3. Select the medication review message to read the results.

If you do not see a message, your review may still be in progress, you may not currently qualify, or there may not be enough information to complete a review.

 

What should I do after receiving a recommendation?

Read the full message in your Hello Heart inbox. The message will explain the pharmacist’s review and whether a change is recommended.

Do not stop, start, or change a medication on your own. Follow the instructions in the message and discuss any medication changes with the appropriate healthcare provider.


What if I changed my mind about opting out?

You can enable the feature at any time in the app by following the steps below.

  1. Open the Hello Heart app and log in.
  2. Tap on the Medications tab.
  3. On the screen that requests connecting your records to get tailored recommendations, select "Connect”.
